Registered Nurse - Step Down

Location: Redlands, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• 1:3 patient ratio with tracheostomy and ventilator management required
• Management of non-titrated cardiac drips including Amiodarone, Milrinone, Lasix, Dopamine, Dobutamine (titrated drips like Cardizem also required)
• Insulin management (3:1 ratio preferred but not required)
• Weekend requirement: minimum 2 weekends per month (may be more)
• Float within areas of competency: med/surg, telemetry, observation
• Use of Omnicell medication system and SBAR communication
• 24-hour orientation provided
• Navy scrubs required (not provided)
• No local candidates within 50 miles considered
• Provider support includes interpretation services, lift teams, pharmacy, phlebotomy, physical therapy, radiology, rapid response teams, respiratory services, social services, unit secretary, intensivist and hospitalist coverage

Required Skills and Experience

• Experience in MS/Telemetry, PCU/DOU/IMC/Stepdown units
• Management and interpretation of dysrhythmias
• Telemetry monitoring on the unit
• Use of Rapid Response Teams
• Post-myocardial infarction care
• Feeding tube care and management
• Pre/post hemodialysis management and renal failure care
• Accessing ports, BPA, central line and peripheral line care/management
• PICC line management and IV starts
• Heparin protocols
• Management of cardiac and vasoactive drips (no titration)
• Care of ventilated patients and tracheostomy management
• Chest tubes (Emerson/Pleur-evac), external CPAP/BIPAP, high flow nasal cannula (HFNC)
• Interpretation of ABGs
• Oxygen delivery devices including nasal cannula and face masks
• Non-invasive airway management
• Isolation precautions
• Wound care and wound vac management

Preferred Skills:

• Post cardiac angiogram care
• IV insulin protocols and titration of insulin drips
• Titration of cardiac drips
• Pre/post neurosurgery care

Certifications Required

• Registered Nurse license valid in California
• NIH Stroke Certification
• ACLS (American Heart Association only; RQI not accepted)
• BLS (American Heart Association only; RQI not accepted)

Preferred:

• Meditech experience

Additional Requirements

• Minimum 2 weekends per month availability
• Must be able to float within competency areas
• First-time travelers with PCU experience and 1:3-4 ratios will be considered
• No local candidates within 50 miles

Compliance and Onboarding Requirements

• Background checks including SS trace, county searches, nationwide, sex offender, and international if applicable
• Drug screening (10 panel including THC) within 30 days of start
• Physical exam within last 12 months
• Immunizations and titers: Tdap, TB test or chest x-ray, Rubeola, Mumps, Rubella, Hepatitis B, Varicella, Flu seasonal vaccination or declination, COVID vaccination plus booster or approved exemption
• Fit mask testing prior to start (no facial hair allowed)
• Various facility-specific acknowledgments and signed documents including abuse reporting, confidentiality, code of conduct, employee handbook, and safety manuals
• Competency exams and medication/pharmacology competency as applicable
• References: 2 professional references within 3 years (not a client requirement for subcontractors)
• Employment application and work history verification

Requirements / Qualifications

• Valid Registered Nurse (RN) license in California
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum 1 year of Stepdown nursing experience, with recent Stepdown experience within the last year
• Telemetry experience
• ACLS certification issued by the American Heart Association
• BLS certification issued by the American Heart Association
• NIH Stroke Scale (NIHSS) certification
• Experience with Rapid Response Teams
• Knowledge and experience with Blood Pressure Administration (BPA)
• Central line care and management
• PICC line management
• Heparin protocols
• Management of cardiac drips without titration
• Management of vasoactive drips without titration
• Care of ventilated patients
• Experience with external CPAP/BIPAP
• Knowledge of cerebrovascular accident (CVA) care
