Job Opportunity: Home Health Registered Nurse

Location: Eureka, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Specialty: Home Health Nursing
  • Minimum Years of Experience: 1 year, recent experience required
  • OASIS and HCHB EMR experience required
  • Average Patients per Day: 5-7 (to be confirmed during interview)
  • Guaranteed Hours: 40 hours per week
  • Types of Cases: Geriatric
  • First-time travelers NOT accepted
  • Weekly minimum productivity: 30 points
  • On-call required (details to be discussed during interview)
  • Territory: Up to a 60-mile radius from the office/hub
  • Locals accepted at travel pay rate
  • Dress Code: Scrubs (any color), no jeans
  • Cell phone use: Not allowed; iPad provided for charting
  • Mileage reimbursed at IRS standard rate
  • All charting must be completed by 9 AM the following morning
  • Timely communication is required
  • Flexibility with territory expected

Required Qualifications

  • Valid Registered Nurse (RN) license in California
  • Minimum 1 year of recent Home Health nursing experience within the last year
  • Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross
  • Experience with OASIS
  • NURSYS Verification (RN, LPN/LVN, AP)
  • Associate of Science in Nursing / Associate Degree in Nursing

Compliance and Background Requirements

  • Background checks including SSN trace, county searches (last 7 years), FACIS Level 1, nationwide sex offender, motor vehicle records, education and work history verification (some verifications may be grandfathered if previously completed)
  • EPLS/GSA/SAM check within 90 days of start
  • HHS/OIG check within 90 days of start
  • Fingerprinting as required by state regulations (specific rules for Alaska, Connecticut, Washington)
  • Education verification of highest degree related to profession
  • State-specific criminal history and abuse registry checks as applicable
  • Drug screening: 9-panel drug test within 12 months of start
  • Physical exam within last 12 months (not required annually)
  • Fit mask test (N95 accepted)
  • Negative TB test or chest X-ray within 1 year (or TB questionnaire if applicable)
  • Immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella
  • Seasonal flu vaccination or facility-specific declination form required during flu season
  • COVID vaccination or facility-specific declination required
  • Proof of current auto insurance required due to driving responsibilities

Additional Requirements

  • 2 professional references within 2 years for travel, allied, or per diem acute care roles
  • 1 professional reference for per diem non-acute care roles
  • Full 7 years of work history documented with no gaps
  • Annual skills checklist and competency exams as applicable
  • Signed orientation safety manual acknowledgment (updated annually)
  • Compliance with agency background attestation and acceptable use policies

Notes

  • Prior travel experience is not accepted for first-time travelers; must have completed at least one prior travel contract
  • Flexibility with territory and timely communication are critical for success in this role
  • All charting and documentation deadlines are strict and must be adhered to
