LPN Case Manager

Overview

• Specialty: Adult/Geriatric Focused
• Setting: Home Health / Mobile Case Management
• Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.
• Shift: Day (5x8 Hours)
• Contract length: 13 weeks
• Weekly hours: 40
• Local candidates accepted: Yes, at a lower rate if under 75 miles
• First-time travelers accepted: Yes

Required Qualifications

• Valid RN license in New York
• Associate of Science in Nursing / Associate Degree in Nursing
• 1 year of Case Management experience
• Case Management experience within the last year
• Care Manager experience
• BLS
• Experience with Epic

Preferred Qualifications

• Clinical knowledge or experience with CHF, COPD, and multiple complex comorbidities
• Community resource knowledge

Role Details

• Supports the Transitions of Care program focused on reducing 30-day readmission rates
• Works under the direction of an RN Care Manager to help implement the care plan
• This is not a hands-on nursing role
• Responsibilities include appointment scheduling, biometric data monitoring, education reinforcement, patient visits as needed, home visits, connecting patients with community resources, frequent patient contact, care team rounds, and other tasks directed by the RN
• May travel during the workday for home visits, hospital visits, patient office visits, and provider offices
• Floating includes Rochester General Hospital, Unity Hospital, patient homes, and provider offices
• Monthly caseload is approximately 25–30 patients
• Care team includes an RN, Social Worker, Pharmacist, and other professionals
• Documentation is completed in the EHR
