Overview

Role: Care Coordinator inpatient
Schedule: Days, Monday–Friday, 8:00am–4:30pm
Contract length: 8 weeks
Setting: Acute hospital setting

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
Case Management (1 year of experience)
Case Management experience within the last year
1 year or more of total travel experience
Experience in an Acute Care setting
Driver's License
Experience with discharge planning
Experience with utilization review (InterQual)
Psychosocial assessment skills
Experience with Epic

Preferred

Additional Notes

Current flu and COVID vaccination/documentation requirements apply
Background check and other onboarding documents are required before start
