Required Qualifications

Valid Physical Therapist license in Oregon
Physical Therapist (1 year of experience)
Physical Therapist experience within the last year
ACLS - Advanced Cardiac Life Support issued by American Heart Association or Heart and Stroke Foundation
BLS - Basic Life Support issued by American Heart Association or Heart and Stroke Foundation
Experience in a(n) Home Health setting
Experience with OASIS

Schedule / Work Expectations

May visit up to 6 patients per day
Standard productivity: 30 points
Occasional daily travel radius may be up to roughly 60 miles
First-time travelers accepted
Weekend work not required
On-call not required
Floating between like units not required
Block scheduling not available
Local candidates accepted within a 50–75 mile radius

Work Environment

Business casual attire; scrubs allowed but not required
Charting includes EPIC Remote Client for Home Health, Rover, and Oasis
Free parking
Mileage reimbursed at IRS standard rate

RTO restrictions should be confirmed with the hiring manager
Facility may call off up to one shift per pay period
