Overview
- Travel/contract role in Salida, California
- Day shift: 5x8 hours, 8:00 AM to 5:00 PM
- 40 hours per week
- 13-week contract
- May float within Stanislaus County, San Joaquin County, and Merced County
Role Summary
- Provides in-home nursing care, including assessment, care coordination, direct patient care, patient/family education, and transitions of care
- Develops and manages an interdisciplinary plan of care and supports collaborative communication with patients, families, physicians, and other healthcare providers
Required Qualifications
- Valid RN license in California
- Home Health (1 year of experience)
- Home Health experience within the last year
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
- Associate of Science in Nursing / Associate Degree in Nursing
Compliance / Pre-Start Requirements
- 7-year background check
- Sex offender search
- Education verification
- Employment application with full 7-year work history and no gaps
- Unit-specific skills checklist
- Physical exam within 1 year of start
- Fit test within 1 year of start
- Negative 9-panel drug screen within 6 months of start
- TB screening within 90 days of start
- Flu vaccination documentation during flu season
- Tdap within 10 years of start
- Color vision screening by an in-person licensed healthcare provider before start
- Candidate review/signature required on assigned orientation and policy documents before start
Additional Notes
- Home health role; personal or auto insurance is required for driving positions in this setting
- Bachelor of Science in Nursing is preferred
