Provides in-home nursing care, including assessment, care coordination, direct patient care, patient/family education, and transitions of care
Develops and manages an interdisciplinary plan of care and supports collaborative communication with patients, families, physicians, and other healthcare providers

Valid RN license in California
Home Health (1 year of experience)
Home Health experience within the last year
BLS - Basic Life Support issued by American Heart Association or American Red Cross
Associate of Science in Nursing / Associate Degree in Nursing

Unit-specific skills checklist
Physical exam within 1 year of start
Fit test within 1 year of start
Negative 9-panel drug screen within 6 months of start
TB screening within 90 days of start

Color vision screening by an in-person licensed healthcare provider before start
Candidate review/signature required on assigned orientation and policy documents before start

Home health role; personal or auto insurance is required for driving positions in this setting
Bachelor of Science in Nursing is preferred
