Contract Length: 13 weeks

Specialty: Therapy/Rehabilitation (Physical Therapist Assistant)
Setting: Home Health
Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days per week
Hours: 40 hours per week guaranteed (36 guaranteed hours weekly after ramp-up)
Patient Load: 5-6 patients per day, 1:1 ratio; productivity standard is 27 points daily
Start Date: ASAP (compliance process takes about 2 weeks)
Weekend Work: Optional to meet productivity goals
On-Call: Not required
Travel: Occasional travel up to 50+ miles radius; own transportation required
Locals Accepted: Yes

Valid Physical Therapy Assistant license in California
Minimum 1 year of Physical Therapy Assistant experience, including experience within the last year
Experience in a Home Health setting
Current BLS (Basic Life Support) certification

Government-issued photo ID (Driver’s License/State ID)
Emergency Contact Form
I9 Attestation
Background Checks including 7-year history, EPLS/GSA/SAM, HHS/OIG (updated annually)
Physical Exam within 1 year, cleared to work without restrictions
Immunizations/Vaccinations:
- Tdap within 10 years or annual declination
- Negative TB test or chest X-ray within 1 year
- Measles, Mumps, Rubella: 2 vaccines or positive titers within required timeframes
- Hepatitis B: Titer within 5 years or vaccine series or declination
- Varicella: 2 vaccines or positive titer within required timeframes
- Flu vaccine or declination required year-round
- COVID-19 vaccine documentation or declination form required
Drug Screening: 10-panel drug screen within 1 year
Auto Insurance and Registration (must be current throughout assignment)
References: 2 professional references within 3 years (travel/allied/per diem acute care)
Work History Verification: 1 year related employment verification required
Skills Checklist and Competency Exams as applicable

First-time travelers are not accepted for this assignment
Guaranteed hours do not apply during the first 2 weeks (ramp-up, holidays, time off)
