Job Description
- Coordinates all aspects of care (inpatient and/or outpatient) per program assignment, including:
- Record review
- Medication refills and/or prior authorizations
- Patient education
- Review of test/procedure/lab results
- Makes referrals for medically necessary services, consults, equipment, and supplies.
- Functions as a liaison between administrative, clinical staff, and physicians within the transplant department and other departments involved in patient care.
Care Provision Responsibilities
- Assists and supports patient discharge planning as needed.
- Provides patient education regarding care and medications.
- Assists patients and/or physicians in clinic and on hospital floors as needed.
- Carries out patient care orders as directed by the physician and in accordance with hospital policy.
- Assists with patient admissions and discharges as required.
Requirements
- BLS Certification
- California RN License
Shift Details
- Monday to Friday, 5x8s (no weekends or holidays)
Contract Length
- 13 weeks
Certifications Required
- ACLS - Advanced Cardiac Life Support
- BLS - Basic Life Support
- PALS - Pediatric Advanced Life Support
- NRP - Neonatal Resuscitation Procedure
- CCRN - Critical Care RN
- TNCC - Trauma Nursing Core Course
- EFM - Electronic Fetal Monitoring
- NIH Stroke Certification
- Additional certifications may be required based on specific job duties.
Health Documents Required
- Physical Exam (within 1 year of start)
- Fit Test (within 1 year of start)
- Immunization records (including Tdap, MMR, Varicella, Hepatitis B)
- Negative TB or Chest X-Ray
- Drug Testing (within 6 months of start)
- Flu Seasonal Vaccination/Documentation (for flu season)
Background and History Checks
- 7 Year Background Check
- EPLS/GSA/SAM checks
- FACIS Level 3 search required within 2 weeks of start
Additional Information
- Candidates must complete a skills checklist and provide professional references.
- Education verification is required, with a preference for a Bachelor of Science in Nursing for RN positions.
- All candidates must have a complete 7 years of work history in their application.
