Employment Type: Long-Term Assignment (LTA)

Work Schedule

• Every Other Week (EOW) / Every Other Weekend (EOH)

Required Certifications & Licenses

• BLS (Basic Life Support)

  • American Heart Association certification required for all direct patient care personnel.
  • Must provide scanned front and back copies with expiration date.
• ACLS (Advanced Cardiovascular Life Support)

  • Required for RNs in CCU, ED, PACU, MICU, SICU.
  • Must provide scanned front and back copies with expiration date.
• NIHSS (National Institutes of Health Stroke Scale)
• Valid Nursing License

  • Verification must be dated within 30 days of start.
  • Upload state verification (e.g., NURSYS for RNs and LPNs).

Required Health & Safety Compliance

• Fit Test
• Trinity 10 Panel Drug Screen

  • Completed within 30 days of start (preferred within 14 days).
  • Must include: Cocaine, Amphetamines, Opiates, 6-Monoacetylmorphine, Propoxyphene, PCP, Barbiturates, Benzodiazepines, Methaqualone, Methadone.
  • No THC allowed.
• COVID-19 Vaccine

  • Proof of September 2025 booster, declination, or exemption (declination not accepted in CA or IL).
• Influenza Vaccine

  • Proof of vaccine, declination, or exemption (declination not accepted in CA or IL).
  • Valid for flu season 10/1/2024 - 4/30/2025.
• MMR (Measles, Mumps, Rubella)

  • Proof of 2 vaccinations with dates or IgG titer lab report.
• Hepatitis B

  • Must have started vaccination series prior to start (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later).
  • Titers not accepted.
• Varicella

  • Proof of 1 vaccination with date or IgG titer lab report.
• 2 Step PPD OR IGRA (Quantiferon/Tspot)

  • Positive TB requires chest x-ray within last year (for MI, IN, CT, MA) or within 5 years for other regions.
• Tdap Vaccine

  • Current, non-expired vaccine required.
  • Declinations accepted if no pediatric interaction.

Required Professional Documentation & Compliance

• Trinity Code of Conduct Form (signed)
• Trinity I9 Form (completed by agency)
• Confidentiality & Information System Security Agreement (signed)
• Integrity and Audit Services Form (signed shortly before start)
• Non-Employee Upload Form (pages 1-3 and 28 completed and uploaded)
• Specialty Specific Competency Testing

  • Copy of specialty exam completed within the last year.
• Physical/Health Statement

  • Documentation from exam within last year confirming fitness to work.
• Criminal Background Check

  • Federal, nationwide, county criminal check for last 7 years, including SSN verification.
  • Must be dated within 30 days of start.
• Education Verification

  • Highest academic degree or diploma/GED verified by 3rd party screening.
• EPLS (Excluded Parties List System) Check

  • Must be dated within 30 days of start.
• OIG (Office of Inspector General) Check

  • Must be dated within 30 days of start.
• Joint Commission CORE Exam

  • Completed within the last year.
• NSO (National Sex Offender) Check

  • Must be dated within 30 days of start.
