Job Opportunity: COTA SNF (Therapy/Rehabilitation)

Location: Lancaster, California
Employment Type: Travel/Contract


Assignment Details

  • Start Date: ASAP (flexible for later starts)
  • Contract Length: 13 weeks (preferred range 13 - 26 weeks)
  • Workweek: 36 hours
  • Shift Schedule:
    • 5 days per week, 8-hour day shifts
    • May include occasional weekend coverage and floating

Requirements

  • Specialty: Therapy/Rehabilitation (Certified Occupational Therapy Assistant - COTA)
  • Experience: Minimum 1 year of experience required; new graduates will be considered
  • Prior Travel Experience: Not explicitly required

Certifications & Licenses

  • ACLS (Advanced Cardiac Life Support) from American Heart Association or American Red Cross
  • BLS (Basic Life Support) from American Heart Association or American Red Cross (current, clear copy required)
  • PALS (Pediatric Advanced Life Support) preferred
  • State License verification as applicable
  • National Tech Certification as applicable (note some states require state licenses for techs)

Compliance & Documentation

  • Government issued photo ID (Driver’s License/State ID)
  • Emergency Contact Form (required prior to start)
  • Background Checks (standard 7-year search, EPLS/GSA/SAM, HHS/OIG; updated annually)
  • Competency Exams as applicable (Medication Test/Pharmacology and general competency exams)
  • Physical Exam within 1 year of start
  • Negative TB test or Chest X-Ray within 1 year of start (with specific guidelines for past positives)
  • Immunizations and Titers:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (Titer, Declination, or Vaccine Series)
    • Varicella (Titer, Vaccines, or history of Chicken Pox)
  • Drug Testing: 9 Panel Drug Screen within 1 year of start (instant drug screen acceptable)
  • Seasonal Flu Vaccination or Declination during flu season (Oct 1 - Mar 31)

Additional Requirements

  • 2 Professional References for Travel, Allied, or Per Diem Acute Care roles (within 3 years of start)
  • 1 Professional Reference for Per Diem Non-Acute Care roles
  • Annual Skills Checklist

  • Cancellation notice: 14 days
