Job Description & Requirements
Minimum Qualifications
- Graduate from an accredited school of Practical Nursing
- Current Practical Nurse license in Illinois or ability to obtain license two weeks prior to starting
- Current Cardiopulmonary Resuscitation (CPR) certification
- Proof of successful completion of medication administration (Pharmacology) coursework as required by Illinois Department of Financial and Professional Regulation (IDFPR)
Preferred Qualifications
- 1 year recent full-time paid clinical experience in an acute care or ambulatory care facility
Experience Required
- Specialty: LVN / LPN
- Years of Experience: 1 year (preferred)
Prior Travel Experience

Work Schedule & Conditions
- Weekend Requirement: Every other weekend (2 weekends per month)
- Return to Office (RTO): Maximum 5 days, no more than 2 consecutive days; no RTO on holidays or weekends, and no RTO the day before or after a holiday
- Mandatory float: Clinicians must float within their scope to meet facility needs
- Guaranteed Hours: 40 hours per week

Certifications & Competencies Required
- Current CPR certification
- Medication Test/Pharmacology Competency Exam (required for starts on/after 4/14/24, must be completed within 12 months of start)
- Color Blind Testing (required for starts on/after 1/16/24, must be completed within 12 months of start)
- Competency Exam (required for starts on/after 4/14/24, must be completed within 12 months of start)
Licenses & Background Checks
- Valid Illinois Practical Nurse license (or ability to obtain)
- Background Check within 12 months covering all locations lived/worked/schooled in past 7 years, updated annually
- EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
- Illinois Healthcare Worker Registry Search (for aides and nursing assistants, not required for LVNs/LPNs)
Health & Safety Requirements
- Physical Exam within 12 months of start, signed by PA, NP, DO, or MD
- Fit Mask test (N95 or equivalent) within 1 year of start, updated annually
- Tetanus/Diphtheria/Tdap vaccine within 10 years or signed declination within 12 months of start
- Negative TB test (QGold or TSPOT) within 12 months of start, updated annually (PPD not accepted after 12/15/2025)
- Required immunizations or titers: Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella
- Negative 9 Panel Drug Screen within 90 days of start, updated annually
- Seasonal Flu Vaccination or approved exemption required during flu season (10/01 - 04/01)

- This is a mandatory float facility; flexibility to work in different units as needed
