- Home health physical therapy role
- Guaranteed hours are based on completed visits; 25 visits equals 40 hours
Required Qualifications
- Valid Physical Therapist license in California
- Experience in an Outpatient setting
- Experience with OASIS
Experience Preferences
- Home health experience preferred
- If no home health experience, outpatient experience is required
Compliance / Pre-Start Requirements
- Government-issued photo ID
- Valid passport before start
- I-9 attestation
- Background check
- EPLS/GSA/SAM check
- HHS/OIG check
- Passport-style photo for badge
- Personal or auto insurance
- Auto registration
- Training content acknowledgment
- Hand hygiene policy acknowledgment
- Physical exam within 12 months of start
- Tdap within the past 10 years
- Negative TB test or chest X-ray within 1 year of start
- Measles, mumps, and rubella immunity documentation as applicable
- Hepatitis B titer or vaccine series
- Varicella immunity documentation
- Negative 9-panel drug screen within 1 year of start
- Seasonal flu documentation
- COVID vaccination or approved exemption
- Employment application
- Skills checklist

- Mileage between patients is reimbursed at the IRS standard rate
