Home health physical therapy role
Guaranteed hours are based on completed visits; 25 visits equals 40 hours

Required Qualifications

Valid Physical Therapist license in California
Experience in an Outpatient setting
Experience with OASIS

Experience Preferences

Home health experience preferred
If no home health experience, outpatient experience is required

Training content acknowledgment

Tdap within the past 10 years
Negative TB test or chest X-ray within 1 year of start
Measles, mumps, and rubella immunity documentation as applicable
Hepatitis B titer or vaccine series
Varicella immunity documentation
Negative 9-panel drug screen within 1 year of start
Seasonal flu documentation
COVID vaccination or approved exemption
