Job Details

• Specialty: Home Health Physical Therapy
• Schedule: Monday to Friday, 8:00 AM to 4:30 PM (5x8 hour shifts)
• Occasional weekends approximately every 6 weeks
• Average caseload: 6-7 patients per day (geriatric cases)
• No on-call required
• Territory: Up to 60-mile radius from the office; flexibility with territory expected
• Dress code: Scrubs (any color), no jeans
• Documentation and communication must be timely
• iPad provided for work; cell phone use not allowed during work
• Mileage reimbursed at IRS standard rate
• Locals accepted at travel rate
• First-time travelers not accepted

Required Qualifications

• Valid Physical Therapist license in Ohio
• Minimum 1 year of recent experience as a Physical Therapist within the last year
• Basic Life Support (BLS) certification (AHA, ARC, or Military accepted)
• Experience with Homecare Homebase (HCHB) EMR system

Background and Compliance Requirements

• Comprehensive background checks required within 90 days of start, including:

  • SSN trace, county searches (last 7 years), FACIS Level 1, nationwide sex offender, motor vehicle records, education and work history verification
  • State-specific searches as applicable (e.g., Ohio BCI and FBI fingerprinting updated annually; fingerprints must be current within 12 months and reprinted if candidate has lived/worked outside Ohio since last prints)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting required for Ohio placements; must be cleared for code 3701.881
• Education verification of highest degree related to profession required
• Employment history verification covering 7 years, with no gaps, must be completed within 90 days of start
• Two professional references required within 2 years of start

Health and Safety Requirements

• Physical exam within the last 12 months (physician statement)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required
• Immunizations or titers required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months of start
• Flu seasonal vaccination or facility-specific declination form required during flu season (Aug-Nov)
• COVID vaccination or facility-specific declination required

Additional Requirements

• Proof of current auto insurance required due to driving responsibilities
• Signed acknowledgment of Orientation Safety Manual (updated annually)
• Completion of facility-specific Acceptable Use Policy prior to start
• Compliance with agency background attestation and facility-specific documentation

Notes

• Prior travel experience is not accepted; candidates must have completed at least one prior travel contract to qualify
• Timely completion of all background checks, fingerprinting, and credentialing is critical for onboarding
• Flexibility with patient territory and schedule is expected for this role
