PT Home Health (Travel/Contract)

Location: San Ramon, California
Contract Length: 13 weeks

Job Overview

• Home Health Physical Therapist covering multiple areas including Vacaville, Fairfield, Rio Vista, Suisun City, Dixon, Vallejo, Benicia, Napa, American Canyon, and Yountville.
• Shift: 8-hour day shifts (8:00 AM – 4:30 PM), 5 days per week.
• Expected to see 5-6 patients per day with a 1:1 patient ratio.
• Guaranteed 36 hours weekly (note: guaranteed hours do not apply during the first 2 weeks, holiday weeks, or time off weeks).
• Weekends are optional for picking up additional patients to meet productivity goals.
• Own transportation is required; occasional travel radius may exceed 50 miles.
• Documentation and timely communication are mandatory; documentation tools will be provided.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum of 1 year of Physical Therapist experience, including experience within the last year.
• Experience in a Home Health setting.
• Current Basic Life Support (BLS) certification (American Heart Association or American Red Cross).

Compliance & Onboarding Requirements

• Government-issued photo ID (Driver’s License/State ID).
• Emergency Contact Form.
• I9 Attestation.
• Background checks including 7-year work/living history, EPLS/GSA/SAM, and HHS/OIG checks (updated annually).
• Physical exam within 1 year prior to start, confirming clearance to work without restrictions.
• Immunizations and health documentation:

  • Tdap vaccine within 10 years or annual declination.
  • Negative TB test or chest X-ray within 1 year.
  • Measles, Mumps, Rubella (MMR) vaccination or positive titers within required timeframes.
  • Hepatitis B titer or vaccine series or declination.
  • Varicella vaccination or positive titer.
  • Seasonal flu vaccine or declination (required year-round).
  • COVID-19 vaccination documentation or facility-specific declination form.
• 10-panel drug screen within 1 year of start.
• Auto insurance and current vehicle registration (must be maintained throughout assignment).
• Facility-specific forms including arbitration agreement, confidentiality agreement, and employee data forms must be completed prior to start.

• Facility uses MediScrib & OASIS documentation systems; training provided.
• Dress code: Any color scrubs.
• Free parking available.
• Mileage reimbursement follows IRS standard rates.
• Locals are accepted for this travel assignment.
• On-call is not required.
