Job Opportunity: Case Manager (Registered Nurse)

Location: Oakland, California
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Work schedule includes weekends (every other weekend)
  • Float position covering all inpatient units
  • Conduct comprehensive assessments of patients’ medical, psychosocial, and discharge planning needs
  • Collaborate with interdisciplinary teams to develop, implement, and update individualized discharge and transition plans
  • Coordinate post-acute services such as SNF, home health, DME, hospice, ensuring safe and timely transitions across levels of care
  • Facilitate communication among providers, patients, families, payors, and community resources to remove barriers to discharge
  • Complete timely and accurate documentation in compliance with regulatory requirements, organizational policies, and quality standards

Required Qualifications

  • Valid Registered Nurse (RN) license in California
  • Minimum 3 years of case management experience, including acute hospital case management within the last year
  • Experience in acute care, home health, hospice, skilled nursing facility (SNF), long term acute care (LTACH), outpatient, and clinic settings
  • Proficient with EPIC electronic health record system
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • Strong skills in utilization management and discharge planning
  • Demonstrated ability to coordinate care across post-acute settings

Required Documents and Compliance

  • Valid photo ID (Driver’s License or Passport)
  • Background checks including criminal, sanction, and exclusion screenings within 90 days prior to start
  • Education verification confirming minimum required education
  • Current and unencumbered California RN license verification (NURSYS) within 15 days prior to start
  • Physical exam within 90 days prior to start, signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform job duties
  • Fit mask test within 90 days prior to start (3M 8210, 3M 1860S, or 3M 1860R masks required)
  • Immunizations and health screenings:
    • Tetanus/Diphtheria/Tdap (within 10 years or signed declination)
    • Negative TB test or chest X-ray updated annually
    • Proof of MMR (Measles, Mumps, Rubella) vaccinations or positive IgG titers
    • Hepatitis B positive titer or signed declination
    • Varicella vaccinations or positive IgG titer
    • Seasonal flu vaccination or signed declination (masking required during flu season)
  • Drug testing: Oral saliva drug screen within 30 days of start (specific analytes required)
  • Completion of facility-specific competencies and orientation modules on first day of work
  • References and employment verification for the past 3 years

Additional Information

  • Prior travel experience is not explicitly required for this position
  • Orientation and competency modules must be completed by 11:59pm PST on the first day of the contract
  • Failure to complete required modules within 14 days of start will result in immediate termination of assignment
