Job Description

• Provide physical therapy services in a Skilled Nursing Facility (SNF) setting.
• Position covers a vacant full-time equivalent (1.0 FTE) role.
• May occasionally float to one other hospital site depending on patient care volume.
• No weekends or on-call required.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 2 years of physical therapy experience, including at least 2 years in a Skilled Nursing Facility (SNF) setting.
• Physical therapy experience within the last year.
• Basic Life Support (BLS) certification.

Certifications & Licenses

• BLS (Basic Life Support) - AHA only.
• California PT license (current and unencumbered).

Required Documents & Compliance

• Valid photo ID (Driver’s License or Passport).
• Background check within 90 days prior to start, including multiple criminal and sanction searches.
• Education verification confirming minimum required education completed.
• Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN, confirming good health and ability to perform job duties.
• Fit mask test within 90 days of start (specific mask types required).
• Immunizations and titers:

  • Tetanus/Diphtheria/Tdap (within 10 years or signed declination).
  • Negative TB test or chest X-ray updated annually.
  • Measles, Mumps, Rubella (proof of 2 MMR vaccinations or positive IgG titers).
  • Hepatitis B (positive titer or completed series or signed declination).
  • Varicella (proof of 2 vaccinations or positive IgG titer).
• Drug testing: Oral saliva drug screen within 30 days of start (specific analytes required).
• Seasonal flu vaccination or signed declination required annually.
• Color blind exam within 90 days of start.

Additional Requirements

• Employment history verification for the past 3 years.
• Current resume including last 7 years of employment.
• Completion of facility-specific competencies and orientation modules on the first day of work.
• Photo for badge (passport style, no filters).

Notes

• Prior travel experience is not explicitly required for this position.
