Job Opportunity: Licensed Practical Nurse (LPN)

Employment Type: Travel/Contract
Contract Length: 8 weeks

Job Details

• Shift: Mix of 3pm-11:30pm and 11pm-7:30am
• Patient Population: Mix of Long Term Care and Rehabilitation patients
• Average Daily Census: 120
• Staffing Ratios:

  • Days: 1:20
  • Nights: 1:40
  • Weekends: 1:20
• Weekend Coverage: Every other weekend (total of 8 weekend shifts during contract)
• Float Requirement: Must float between all 3 units in the building
• Electronic Health Record: Epic required
• First Time Traveler Friendly: Yes
• Local Candidates: Accepted if living more than 75 miles away

Required Qualifications

• Valid LPN / LVN license in New York
• Minimum 1 year of LPN experience in a Long Term Care setting within the last year
• Skilled Nursing Facility / Nursing Home experience
• Experience with Epic Electronic Health Record system
• Certifications:

  • Basic Life Support (BLS) issued by American Heart Association
  • Advanced Cardiovascular Life Support (ACLS) issued by American Heart Association
  • Pediatric Advanced Life Support (PALS) issued by American Heart Association
  • Trauma Nursing Core Course (TNCC)
  • Electronic Fetal Monitoring (EFM)
  • Tech Cert Verification - National
• Orientation Safety Manual Acknowledgement
• State License 1 Verification
• NURSYS verification
• National Institutes of Health Stroke Scale (NIHSS) certification

Compliance & Documentation Requirements

• Government-issued photo ID (Driver’s License or State ID)
• Background checks including:

  • 7-year criminal background check (felony checks)
  • EPLS/GSA/SAM and HHS/OIG checks
  • Education verification from primary source
  • Foreign equivalency evaluation if applicable
• Primary source verification of state license within 30 days of start
• NURSYS verification within 30 days of start
• Physical exam within 1 year of start, updated annually
• Fit mask test within 1 year of start, updated annually
• Negative TB test or chest X-ray within 1 year of start
• Immunizations and titers: Measles, Mumps, Rubella, Hepatitis B, Varicella (with specific requirements for titers and vaccines)
• Drug screening within 30 days of start
• Seasonal flu vaccination or signed declination during flu season
• Snellen eye exam within 1 year of start
• COVID vaccination copy preferred but not required
• Medication/pharmacology competency exam as required
• Color blindness testing (one-time)
• Competency exams as required by specialty
• Orientation safety manual acknowledgement
• Photo for badge (passport-style with solid background)

• Candidates must complete all health-related requirements and lab work within 5 days of signing the contract.
• References: 2 references within 3 years required for agency compliance (not facility requirement).
• Employment application and work history verification required.
• New York State Nurse Aide Registry registration required within 3 months for Long Term Care.
• Fingerprinting and disciplinary action searches may be required depending on unit assignment.
• Sex offender registry check required within 30 days of start.
• Medicaid exclusion list check required within 3 months of start and monthly thereafter.
