Position Details

Required Credentials and Documentation

Identification

• Current and valid Driver's License, government photo ID, or passport.
• Proof of name change if applicable (certifications and licenses must have current legal name).

Background Checks (Required within 30 days of start)

• 7-year background check (renew annually).
• EPLS/GSA/SAM check.
• HHS/OIG check.
• Education verification (3rd party verification of highest education level).

Certifications (AHA certificates only; certificates of completion NOT accepted)

• ACLS (Advanced Cardiac Life Support) – required for ER, PCU, CVICU, ICU, Cath Lab, LAD, IR, Urgent Care.
• BLS (Basic Life Support).
• PALS (Pediatric Advanced Life Support) – required for ER, NICU, PICU, Pediatrics, Interventional Radiology, Urgent Care.
• NRP (Neonatal Resuscitation Procedure) – required for Nursery, NICU, L&D, Mother/Baby.
• TNCC (Trauma Nursing Core Course) – required for ER.
• EFM (Electronic Fetal Monitoring) – AWHONN Intermediate required for L&D; AWHONN Advanced also accepted.
• Technical Certification Verification (National) – required for Certified OR Tech/Surgical Tech, Radiology Tech, Phlebotomist.
• State License Verification – required within 30 days of start; temp licenses NOT accepted.
• NURSYS Verification (for RN, LPN/LVN, AP) – required within 30 days of start.
• CPI (Crisis Prevention Institute) – preferred for Psych.

Competencies

• Dysrhythmia/EKG Competency Exam – preferred for Tele, Stepdown, PCU, ICU, ER (80% pass rate).

Facility-Specific Documents

• OSHA Form – required for onsite fit testing.

Health Requirements

• Physical Exam – within 1 year of start; must state good health and ability to perform job duties; signed by MD, NP, or PA.
• Fit Mask Test – quantitative fit test with 3M mask within 90 days prior to start (qualitative tests not accepted); must be renewed annually; religious exemptions evaluated case-by-case; not required for certain lab and tech roles.
• Immunizations/Vaccinations:

  • Tetanus/Diphtheria/Tdap – preferred within 10 years or signed declination.
  • Negative TB or Chest X-Ray – negative TB testing required and must be renewed annually with specific testing options.
  • TB Questionnaire – required to support past positive TB; renewed annually.
  • Rubeola (Measles), Mumps, Rubella, Varicella – required positive IGG titer or 2 vaccines; boosters required for low/equivocal titers.
  • Hepatitis B – required positive IGG titer, 3-dose vaccine series, or declination within last year.
• Drug Testing – negative drug screen within 30 days of start; must be renewed annually; includes a comprehensive panel of substances.
• Flu Seasonal Vaccination/Documentation – required October 31 to March 31; declinations and exemptions accepted with proper documentation.
• COVID-19 Vaccine – required; medical and religious exemptions accepted with documentation; specific attestation process for agency candidates.

Employment History and References

• 2 references required, including a direct supervisor reference within the last year.
• Employment application covering 7 years required, with explanations for gaps over 30 days.
• Employment verification preferred for last 5 years.
• Skills checklist required as applicable.
