Job Opportunity: RN Telemetry I

Location: Chicago, Illinois
Facility: Saint Mary Of Nazareth Hospital
Hospital System: Prime Healthcare Foundation, Inc.
Employment Type: Long-Term Assignment (LTA)
**Contract Length: 12 weeks


Unit Details

  • 37-bed Cardiac telemetry unit
  • Typical nurse-to-patient ratios: 1:5, 1:6; rarely 1:7
  • Must be able to float to other medical/surgical, telemetry, and orthopedic units

Required Certifications & Licenses

  • BLS (American Heart Association or Red Cross)
  • ACLS (American Heart Association or Red Cross)
  • NIHSS Certification
  • Valid Nursing License (Nursys verification)

Required Exams & Testing

  • Prime Basic Pharmacology Exam (minimum 80% score)
  • Prime Specialty Exam (minimum 80% score)
  • Prime Core Competencies Exam (minimum 80% score)
  • Prime Skills Checklist (submission required)

  • Prime Education Verification
  • Driver’s License or State ID or Passport
  • Prime OIG Check
  • Prime SAM/EPLS (Excluded Individual) Search
  • Prime Criminal Background Check (within one year)
  • Prime Badge Photo (clear headshot)
  • Prime Form I-9 (Employment Eligibility Verification)
  • Prime Job Description acknowledgment
  • Prime Agency Attestation
  • Prime Resume and two professional references (submission required)
  • Social Security Card or Passport (for verification)

Health & Immunization Requirements

  • COVID-19 Vaccine: 2 doses plus booster or declination completed
  • MMR: Proof of 2 vaccines or positive titer showing immunity
  • Hepatitis B: Vaccination or immunity proof (declinations not accepted at some locations)
  • Influenza Vaccine: Required annually during flu season (declinations not accepted at some locations)
  • Varicella: Vaccination or immunity proof
  • Tdap: Vaccination required (declinations not accepted at some locations)
  • TB Test: Required annually
  • Annual TB Questionnaire
  • Positive PPD with Negative Chest X-Ray (optional)
  • Annual Respirator Fit Test (if applicable)
  • Physical/Health Statement: Must be completed annually or within 60 days of start date

Drug Screening

  • Minimum 10-panel drug screen including THC (with specific requirements for California locations)

Additional Mandatory Training & Compliance

  • DCI/BVP (Workplace Violence Prevention Certification)
  • Essentials Onboarding Packet completion
  • Onboarding Education Attestation (education modules must be completed prior to clearance)
  • Prime Color-blind Test (annual requirement)

Notes

  • Must be able to float between units as needed
  • All credentials must be current and verified prior to onboarding
  • Some immunization declinations are not accepted depending on facility policies

This position is ideal for RNs with telemetry experience who are flexible and meet comprehensive credentialing and health requirements.
