Registered Nurse (Medical/Surgical) - Indiana, Evansville

Position Details

  • Unit: Renal/Diabetic Unit
  • Patient Ratio: 1:6-8
  • Beds: 23
  • Shift: 06:45 AM – 7:15 PM
  • Contract Length: 13 weeks
  • Employment Type: Long-Term Assignment (LTA)

Required Qualifications

  • Valid RN license in Indiana
  • Minimum of 1 year of Medical/Surgical nursing experience, with recent experience within the last year
  • Basic Life Support (BLS) certification issued by the American Heart Association
  • Successful completion of Ascension Pharmacology Exam
  • Successful completion of Ascension Core Competencies
  • Successful completion of Ascension EKG Exam
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Health and Safety Requirements

  • Proof of immunizations or immunity for:
    • MMR (two vaccines or positive titer; no declinations)
    • Varicella (two vaccines, serological evidence, or verified history; no declinations)
    • Hepatitis B vaccination series completion or signed declination
    • Tdap vaccine valid within 10 years (no declinations accepted)
  • Influenza vaccine recommended during flu season (Oct 1 – Mar 31); declinations accepted with mask requirement
  • Tuberculosis screening:
    • 2-step PPD or Negative IGRA within 90 days prior to start
    • If positive, negative chest X-ray within 1 year and annual TB questionnaire
  • Fit test for respiratory protection, preferably completed prior to start; updated annually
  • Health statement or physical exam confirming good health prior to first assignment, updated annually
  • 12-panel drug screen within 30 days prior to start, updated annually; retesting required if break in service > 90 days

Documentation and Verification

  • Submission of professional resume/profile and at least two professional references prior to assignment
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 90 days prior to start; updated annually or after break in service > 90 days
  • Employment verification for the past seven years; updated annually or after break in service > 90 days
  • Education verification relevant to clinical work within 90 days prior to start; updated after break in service > 90 days
  • Driver’s license or state ID (valid, not expired, matching address requirements)
  • Signed assignment agreement and confirmation of assignment on agency letterhead with accurate dates
  • Badge photo (forward-facing headshot with white or blue background, no alterations)

  • Prior travel experience is not explicitly required
  • Onboarding includes completion of nursing contingent worker orientation and related attestations
  • Optional: Unit-specific checklist and post-assignment hiring manager evaluation may be requested
