Job Description

• Every other weekend coverage
• Teamwork is essential
• Must have current BLS, ACLS, and NIH certifications prior to starting

Required Certifications & Training

• BLS (American Heart Association only) – Required before start
• ACLS (American Heart Association only) – Required before start
• 8 Hours of Stroke Training – Required for permanent staff in related units; includes NIH Stroke Scale (NIHSS) training
• Arnot Core Competency Exam – Must score 80% or higher on Core Comp I, II, and III for RNs
• Fit Test – Must be completed prior to start and within the last 12 months; specific approved mask models required
• COVID-19 Vaccine – Optional; proof of vaccination or agency declination accepted
• Influenza Vaccine – Required or agency-specific declination; if declined, mask must be worn at all times

Licenses & Verifications

• State Nursing License Verification – Must be completed within 30 days prior to start
• Driver’s License or State ID – Valid, unexpired US license or ID required
• Agency Background Check – Must be run within 30 days of start, including SSN address trace and county criminal searches for all counties lived/worked in last 7 years
• Agency Drug Screen – Must be run within 30 days of start; marijuana testing is prohibited per New York state regulations

Health & Immunization Requirements

• MMR (Measles, Mumps, Rubella) – Positive IgG titer or 2 vaccine series required; documentation must show individual results
• Varicella – Positive IgG titer or 2 vaccine series required; documentation must show dates and results
• Hepatitis B – Positive antibody titer, 3 vaccine series, or facility-specific declination form required
• TB Test – Annual requirement; acceptable tests include 2 PPD skin tests (one within 12 months, one within 3 months) or 1 Quantiferon/Spot test within 12 months; past positives require chest x-ray within 12 months
• Tdap – Preferred but not required; if no record, facility-specific declination form required
• Physical/Health Statement – Must be from within the last 12 months, including height, weight, vitals, Snellen eye chart, and a statement of clearance with no restrictions

Professional Requirements

• Arnot Skills Checklist – Must be fully completed and updated annually for the assigned unit
• Arnot Resume – Must document all employment and gaps for the last 7 years, including education history with degree titles and graduation dates
• Professional References – One supervisor reference from the last 12 months and one supervisor or peer reference from the last 3 years
• Employment Verification – Verification of most recent employer required (agency counts as one employer)
• Job Description Acknowledgement – Signed and dated
• Color Headshot – Recent, clear photo with solid background, no filters or sunglasses
