Pharmacist – Long Term Care (LTC)
Location: West Burlington, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: Day shift, 8:30 AM – 6:00 PM, Monday through Friday (no weekends or holidays)
Job Description
- Dispense medications prescribed by physicians and other health practitioners.
- Provide patients with information about medications and their proper use.
- Advise healthcare providers on medication selection, dosage, interactions, and side effects.
- Deliver safe and effective pharmaceutical care while collaborating with support staff.
- Previous Long Term Care pharmacy experience preferred.
Required Qualifications
- Valid Pharmacist license in Iowa.
- Minimum of 2 years of experience as a Pharmacist.
- Pharmacist experience within the last year.
Onboarding and Compliance Requirements
Candidates must complete and provide documentation for the following prior to or at onboarding:
Immunizations and Health Screenings
- Influenza vaccine (or facility declination) annually by December 1 (season Sept 1 – March 31).
- COVID-19 vaccine (declination accepted).
- MMR: 2 vaccines or positive titer; if low/equivocal, 1 booster or declination accepted.
- Hepatitis B: 2 vaccines or positive titer; if low/equivocal, 1 booster or declination accepted.
- Varicella: 2 vaccines or positive titer; if low/equivocal, 1 booster or declination accepted.
- Tdap: Valid within 10 years; declinations accepted.
- TB Test: Negative test within past year (TSPOT or QuantiFERON IGG). If history of positive TB test, must provide documentation including chest x-ray and questionnaire.
- Fit Test for approved respirators (e.g., Moldex 1510, 3M 1860 series) completed prior to start.
- Health Statement from a physician within the last 12 months.
- Color Vision test.
Background and Verification
- Background check within 30 days of start, including SS trace and county search for last 7 years.
- Adult and Child Abuse clearance per state requirements.
- Education verification through a third party; if unverifiable, diploma and/or transcripts required.
- EPLS/GSA/SAM HHS/OIG screening within 30 days of start.
- Drug screen within 30 days of start (9-panel, excluding THC). Instant drug screen accepted if lab results pending.
- Skills checklist completed within the last 12 months.
- SING, NSO, and OFAC screenings within 30 days of start.
- I-9 attestation on agency letterhead.
- Contract employee data sheet.
- Pre-orientation computer-based learning (CBL) acknowledgment form.
Documentation to Submit
- Resume or professional profile.
- Two professional references (one must be a supervisor).
Additional Notes
- Work schedule is strictly weekdays with no weekend or holiday shifts.
- Prior Long Term Care pharmacy experience is preferred but not explicitly required.
