Licensed Practical Nurse (LPN) - Evenings
Location: West Burlington, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 2:30 PM – 11:00 PM
Job Description
- Provide routine daily nursing care and other services based on each resident’s assessment, preferences, and care plan.
- Collaborate closely with the RN Manager to manage day-to-day operations.
- Work in a long-term care setting with a total of 160 beds.
- Resident-to-nurse ratios will be discussed during the interview.
- Previous experience with Epic electronic health records within the last 12 months in a long-term care setting is required.
Required Qualifications
- Current LPN license valid in Iowa.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (AHA or ARC accepted).
- Minimum years of LPN experience: Not explicitly stated (assume standard LPN competency; no conflicting data).

- Current driver’s license or state ID (valid throughout assignment).
- Background check within 30 days of start including SS trace and county search for last 7 years.
- Adult and Child Abuse clearance as required by state.
- Education verification through third party or combined with diploma/transcripts.
- EPLS/GSA/SAM HHS/OIG screening within 30 days of start.
- Drug screen within 30 days of start (9-panel, excludes THC; instant accepted if lab results pending).
- Social Security Number verification within 30 days of start.
- Resume or professional profile submission.
- I-9 attestation completed on agency letterhead.
- Color vision test within 1 year.
- Contract employee data sheet.
- National Sex Offender Registry check within 30 days.
- OFAC screening within 30 days.
- Pre-orientation computer-based learning (CBL) acknowledgment form.
