Job Opportunity: MSW - PCLS (Clinical Social Worker)

Job Description

• Provide social work services for patients on legal holds (e.g., 5150, 5585, 5250, TCON, PCON).
• Conduct initial assessments including Social Determinants of Health (SDOH) for admitted patients.
• Evaluate patients’ mental status, living situation, support system, drug and mental health history, physical functional status, financial status, and discharge planning needs.
• Complete EPIC SW Consults related to PCLS issues.
• Communicate regularly with nursing management and unit social workers/case managers regarding patient status, legal hold expiration, and discharge planning.
• Maintain contact with agencies and families for collateral information and discharge arrangements (non-medical).
• Monitor psychiatric outpatient follow-up needs post-discharge.
• Monitor and manage legal hold transitions and advisements.
• Assess potential for conservatorship for patients on legal holds with grave disability (GD) status and ensure timely referrals.

Required Qualifications

• Master’s degree in Social Work (licensed not required).
• Minimum 2 years of clinical social work experience (higher value from description).
• Previous counseling experience preferred.
• Mental health crisis intervention experience.
• Strong communication skills in English (written, verbal, telephone).
• Ability to work independently with minimal supervision.
• Proficient with computers and office equipment.
• Ability to manage complex interpersonal relations and work under pressure.
• Critical thinking, problem-solving, organizational, and multitasking skills.

Additional Information

• Must be able to type 30 words per minute without errors.
