Clinic Nurse Opportunity

Location: Modesto, California
Employment Type: Clinical Nursing
Contract Length: 26 weeks (approximately 6 months)

Job Summary

Under the direction of the Primary Care Provider (PCP) and supervision of the Clinic Supervisor, the Clinic Nurse provides nursing care to clinic participants, including:

• Supporting clinic overflow
• Checking vital signs
• Performing point-of-care (POC) testing
• Managing and administering medications
• Supporting wound care and other clinical needs within scope of practice

The nurse will manage ongoing care, assist with complex clinical situations and procedures, and provide clinical support to ensure efficient clinic operations. Collaboration with colleagues and stakeholders to promote team culture and mission is essential.

Duties and Responsibilities

Clinical

• Perform and document ordered procedures and medication management as directed by PCP
• Monitor participant condition and promptly update RN Case Manager or clinic staff of changes
• Support care transitions between hospital, skilled nursing facilities, and home settings
• Dispense medications and provide participant education on medication use
• Conduct home visits and provide teaching on disease processes, self-care, and prevention
• Provide backup coverage for other nursing functions as assigned
• Perform PGX testing as directed
• Support Medical Assistants with clinical procedures
• Process Specialty Pharmacy medication orders and escalate issues to MD
• Address medication questions and incidents, ensuring follow-up
• Coordinate medication order changes with providers
• Perform pre-enrollment medication evaluations
• Provide coverage as Center Nurse, Meds MA, and Case Manager as needed

Administrative

• Maintain clinical trackers (e.g., TB Screening, Flu, Pneumovax, Shingles)
• Monitor and restock Emergency Cart monthly
• Stock Meds ATM and manage medication destruction logs

Required Skills & Experience

• Minimum 1 year nursing experience in a clinical setting with frail or elderly populations
• Nursing knowledge and skills to manage complex clinical situations involving seniors
• Ability to work independently, prioritize tasks, and make sound decisions in emergencies
• Excellent clinical, organizational, and communication skills with seniors, families, and interdisciplinary teams
• Experience working with diverse populations
• Highly motivated and self-directed in a fast-paced environment

Education & Licensure

• Associate's degree in Nursing from an accredited program (required)
• Unencumbered RN License (required)
• BLS certification (required)

Preferred Qualifications

• Bilingual English/Spanish (preferred)
