Job Opportunity: Physical Therapist (Home Health)

Location: Beaver Dam, Wisconsin
Contract Length: 13 weeks

Job Details

• Specialty: Home Health Physical Therapy
• Setting: Home Health
• Typical Caseload: Approximately 5 patients per day (varies by visit/evaluation type)
• Patient Types: Geriatric, Joint Replacement, Stroke, CHF/COPD, Parkinson’s
• Work Hours: 8:00 AM – 4:30 PM, self-scheduling available
• Weekend/Holiday Requirements: None
• Dress Code: Solid color scrubs (any color), closed-toed shoes

Required Certifications & Skills

• BLS Certification (required)
• Experience with OASIS documentation
• Lymphedema certification is a plus
• Rehab aides (PTAs) available and supervised by PT

Orientation & Training

• Computer-based training
• 1:1 shadowing in the field

Notes for Candidates

• This position involves providing physical therapy services in patients’ homes.
• Candidates should be comfortable managing a varied caseload including complex diagnoses such as stroke and Parkinson’s.
• Self-scheduling allows flexibility within the stated work hours.

Required Qualifications

• Valid Physical Therapist license in Wisconsin
• Minimum of 2 years of Physical Therapist experience, including experience within the last year
• BLS Certification
• Experience with OASIS documentation
