Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 36 or 48 hours per week
Start Date: January 5 (Non-negotiable)

Required Credentials & Documentation

Licenses & Professional Credentials

• Current nursing license with primary source verification within 30 days prior to start
• Nursys verification within 30 days prior to start
• Driver's License or State ID (must not be expired and address must match requirements)
• Resume/Profile submission required
• At least two professional references prior to first assignment
• Criminal background check including Social Security Trace, Employment Verification, OIG Search, National Sex Offender, and others (updated annually or if break in service > 90 days)
• Employment verification for the past 7 years (updated annually or if break in service > 90 days)
• Education verification relevant to clinical work (completed within 90 days of start)

Certifications

• BLS (American Heart Association) – required before onboarding
• NIH Certification (Group A updated yearly; Groups B-F every 2 years)
• Tdap vaccine valid within 10 years (no declinations accepted)

Health & Immunizations

• MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
• Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
• Hepatitis B: Documentation of vaccination series completion or signed declination
• Influenza vaccine for flu season (Oct 1 - Mar 31); declinations accepted with mask requirement
• Fit Test for respiratory protection (preferred before start, can be done onsite)
• 2 Step PPD or IGRA testing for TB (with follow-up requirements)
• Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire
• Health statement or physical exam prior to first assignment, updated annually
• 12 Panel Drug Screen within 30 days prior to start, updated annually

Testing & Onboarding Requirements

• Ascension Medical Surgical Exam: Minimum 80% score, max 2 attempts, updated every 3 years
• Ascension Core Competencies: Includes HIPAA, Joint Commission, OSHA topics; minimum 80% score, updated yearly
• Ascension EKG Exam: Minimum 80% score, max 2 attempts, updated as required
• Ascension Cerner Test-Out: Cerner test results and training checklist with signed attestation
• Ascension Skills Checklist: Clinical skills checklist completed within one year of start, updated annually
• Ascension Onboarding Requirements Education Attestation (6 hours pre-hire training)
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Nova StatStrip pre-hire modules (required for non-allied positions)

• Ascension Badge Photo: Forward-facing headshot with white or blue background, no alterations
• Confirmation of Assignment documentation upon initial assignment and extensions
• Ascension EPLS (Excluded Parties List System) check within 90 days of start
• Optional: Positive PPD documentation and annual TB questionnaire updates
• Optional: Hiring Manager Evaluation post-assignment
• Optional: Legal documentation for name changes if applicable

This position requires strict adherence to credentialing, immunization, and testing protocols prior to and during the assignment. All documentation must be current and compliant with Ascension Health standards.
