RN Case Manager - Wisconsin (Racine)

Job Details

  • Employment Type: Long-Term Assignment (LTA)
  • Contract Length: 12 weeks
  • Shift: 8:00 AM – 4:30 PM
  • Location: Racine, Wisconsin
  • Role Summary:
    Provide RN case management with care management experience. May be required to float onsite within skill set or to other sites within a 40-mile radius as needed.

Required Qualifications

  • Valid RN license in Wisconsin
  • Minimum of 1 year of case management experience within the last year
  • Care management experience
  • BLS certification issued by the American Heart Association
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Valid Driver’s License

Additional Information

  • May be required to float to other sites within 40 miles based on operational needs.
  • All required documentation and verifications must be current and compliant prior to assignment start.
  • Some optional documents and evaluations may be requested post-assignment or during onboarding.

This position is suitable for experienced RN Case Managers with care management skills who are flexible to work across multiple sites within a regional area.
