RN Case Manager - Wisconsin (Racine)

Job Details

Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks
Shift: 8:00 AM – 4:30 PM
Location: Racine, Wisconsin
Role Summary:
Provide RN case management with care management experience. May be required to float onsite within skill set or to other sites within a 40-mile radius as needed.

Required Qualifications

Valid RN license in Wisconsin
Minimum of 1 year of case management experience within the last year
Care management experience
BLS certification issued by the American Heart Association
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Valid Driver’s License

Additional Information

May be required to float to other sites within 40 miles based on operational needs.
All required documentation and verifications must be current and compliant prior to assignment start.
Some optional documents and evaluations may be requested post-assignment or during onboarding.

This position is suitable for experienced RN Case Managers with care management skills who are flexible to work across multiple sites within a regional area.
