Required Qualifications

• Valid RN license in Connecticut
• Med Surg / Telemetry experience within the last year
• 1 year of Med Surg / Telemetry experience
• BLS issued by American Heart Association
• ACLS issued by American Heart Association
• Associate of Science in Nursing / Associate Degree in Nursing
• Resume
• 2 professional references

Onboarding Requirements

• License/certification verification dated within 30 days of start
• Criminal background check dated within 30 days of start
• Education verification
• EPLS check dated within 30 days of start
• GSA/OIG check dated within 30 days of start
• NSO check dated within 30 days of start
• System access form completed one week before start
• Code of conduct form
• I-9
• Confidentiality and information security agreement
• Conflict of interest disclosure form
• Integrity and audit services form
• Joint Commission testing
• Respirator evaluation
• Tdap within the past 10 years
• COVID-19 vaccine documentation, declination, or exemption
• MMR documentation
• Varicella documentation
• Hepatitis B documentation or declination
• TB screening
• Fit test
• Drug screen within 30 days of start
• Physical/health statement within the last year

Notes

• Influenza vaccine is optional
• Declinations are not accepted for COVID-19 or influenza in CA or IL
• Tdap declinations may be accepted if the site allows and the assignment does not require pediatric interaction
