Required Qualifications

• Valid RN license in Illinois
• Home Health (1 year of experience)
• Home Health experience within the last year
• BLS
• Associate of Science in Nursing / Associate Degree in Nursing
• Driver's License

Preferred Experience

• OASIS experience
• Start-of-care experience
• Strong med-surg experience with some home health experience may be considered

Role Overview

• Provides comprehensive nursing care in the home setting
• Completes initial and ongoing assessments, plan-of-care updates, discharge planning, and documentation
• Supervises CNAs and coordinates care with other healthcare workers
• Completes OASIS data
• Provides patient and caregiver education
• Supports telehealth/virtual visits in addition to home visits

Work Setting Details

• Home health territory is within 15 miles, with possible expansion if census is low
• Typical caseload is 24–26
• Mileage is reimbursed at the standard IRS rate
• Orientation is 2 days, with the first day in the office and the second day in the field
• An iPad is provided for the EMR system
