Social Worker (LCSW) - Temporary Position

Location: Robbinsdale, Minnesota

Hours per day: 8

Job Description

• Provide social work services to patients and their families to assist with adjustment to illness, injury, or hospitalization.
• Collaborate with the healthcare team to address social, economic, emotional, and family factors affecting patient care and recovery.
• Facilitate timely and appropriate discharge planning, including coordination with next level of care and transmission of care plans.
• Identify and coordinate community services for patients in need.
• Participate in discharge planning, including screening and coordinating referrals to nursing homes, assisted living, group homes, hospice, and other facilities.
• Assess patient and family risk factors, support systems, and coping abilities.
• Provide interventions related to child abuse/neglect, domestic violence, guardianship, foster care, adoption, mental health placement, and adult/elderly abuse.
• Support patients and families with emotional, social, and financial challenges related to illness or disability.
• Serve as a resource for palliative care and end-of-life decision support.

Required Qualifications

• Valid Licensed Clinical Social Worker (LCSW) license in Minnesota.
• Minimum of 2 years of experience as a Licensed Clinical Social Worker, including experience within the last year.
• Experience with discharge planning.
• Proficiency with Epic electronic medical records system.

Contract Length

• Approximately 10 weeks.

Additional Information

• Dress code: Business casual.
• Nonbillable hours: 4 hours.
• Strict compliance deadline for documentation submission: 4 pm on the Wednesday before start date (no exceptions).
• Prior work history with the health system must be listed on the resume if applicable.
