Role Overview

• Travel/Contract RN position in Lakewood, California.
• Day shift, Monday through Friday, 7:30 AM to 4:00 PM.
• Must be flexible to start earlier on rare occasions to accommodate the night shift and may need to work a weekend shift during flu season.
• Reports directly to the Chief Human Resource Officer at Lakewood Hospital.

Contract Length

• 25 weeks

Required Qualifications

• Valid California RN license
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Occupational Health experience within the last year
• 1 year of Occupational Health experience
• NIHSS
• Copies of Continuing Education taken within the last 24 months
• Unit Specific Comp Exam
• Medication Exam
• Infection Control/Bloodborne Pathogens exam
• 2 references within 1 year of start in the current specialty
• Employment application showing 1 of the last 2 years in specialty experience at a US facility
• Government issued photo ID
• Criminal Background Check within 1 year of start, updated annually
• Physical exam within 1 year of start, updated annually
• N95 mask fit testing within 1 year of start, renewed annually
• Negative TB test or chest x-ray within 1 year of start
• Hepatitis B series or declination
• Tetanus/Diphtheria/Tdap
• Flu vaccine by 12/5 or signed declination
• Signed HIPAA acknowledgement
• Signed unit-specific job description
• Signed confidentiality agreement within 72 hours of signing
• Signed traveler acknowledgement
• Signed Lakewood orientation packet
• Signed Lakewood quick start/annual reorientation sign-off sheet
• Signed Lakewood CA Abuse and Neglect Reporting Act acknowledgement
• Signed Lakewood contract staff demographics and emergency contact information

Preferred

• Hepatitis C titer is strongly preferred

Additional Notes

• Candidates who decline flu vaccination must follow seasonal masking guidelines.
• Onsite flu vaccines are available.
