Physical Therapist - Home Health (Travel/Contract)
Location: San Jose, California
Contract Length: 16 weeks (approximately 4 months)
Job Overview
- Specialty: Therapy/Rehabilitation (Physical Therapist)
- Employment Type: Travel/Contract
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 5:00 PM)
- Weekly Hours: 40 hours guaranteed
- Patient Load: Minimum 23-24 visits per week required
- Patient Population: Home Health, Wound Care, Geriatric Home Health Patients
- Weekend Work: Not required
- On-Call: Not required (discretionary but not mandatory)
- Locals Accepted: Yes, within a 50-mile radius
Required Qualifications
- Valid Physical Therapist license in California
- Minimum 2 years of experience as a Physical Therapist
- Physical Therapist experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
- Advanced Cardiovascular Life Support (ACLS) certification
- Pediatric Advanced Life Support (PALS) certification
- Tech Cert Verification - National
Hiring & Compliance Process
- Hiring includes phone interviews with the hiring team
- All required compliance documents must be completed and approved before starting patient care (may take 2-3 weeks)
- Required documents include:
- Government-issued photo ID (Driver’s License/State ID)
- Background checks (including SS Trace, County searches, Nationwide, Sex Offender, EPLS/GSA/SAM, HHS/OIG) updated annually
- Certifications: BLS (required), ACLS and PALS preferred but not mandatory for this role
- Physical exam within 6 months signed by MD/DO confirming freedom from active communicable diseases
- Immunizations and titers: COVID vaccine (or approved medical/religious exemption), Flu vaccine or declination form, Tetanus/Diphtheria/Tdap within 10 years or exemption, Measles, Mumps, Rubella, Hepatitis B, Varicella (per policy)
- Negative TB test or chest X-ray within 1 year (with specific rules for past positives)
- Drug screening (9-panel) within 1 year
- Emergency contact form
- Auto insurance card (if applicable)
- References: 2 professional references within 3 years (for travel, allied, per diem acute care)
Work Environment & Expectations
- Charting system: Kinnser/Wellsky; all charting must be completed onsite, no remote charting allowed
- Mileage reimbursed per IRS standard from first to last patient visit (not from/to home)
- Free residential parking available
- Territory/County coverage details discussed during interview; may include surrounding counties
- Qualities valued in clinicians: patient-focused care, punctuality, self-starter attitude, and timely completion of charting (tablet provided for charting)
Additional Notes
- Contract length is 4-6 months; less experienced clinicians (under 2 years) must commit to 6 months (non-negotiable)
- No weekend or mandatory on-call shifts
- COVID vaccination required with allowance for religious exemption; flu vaccination required or declination form during flu season
- Locals within 50 miles are eligible to apply
