Job Opportunity: Medical Surgical LVN / LPN (Travel/Contract)

Location: Memphis, Tennessee
Contract Length: 13 weeks

Position Details

• Specialty: LVN / LPN
• Unit: Medical Surgical
• Shift: Night shift (3 shifts of 12 hours each, 7:00 PM to 7:00 AM)
• Patient Ratio: Approximately 1:12 (team nursing model with support from PCCs, RNs, charge nurse, medication nurse)
• On-Call: As needed
• Weekend Requirement: As needed (no weekend block scheduling, but weekend blocks may be considered)
• Start Date: ASAP
• Locals: Not accepted (strict 100-mile radius rule)

Required Qualifications

• Valid LPN / LVN license in Tennessee
• Minimum 2 years of LPN / LVN experience, including experience within the last year
• One completed travel assignment
• IV Certification
• BLS certification issued by American Heart Association or American Red Cross
• Experience working with Level I Trauma patients
• Medical Surgical experience

• Level I Trauma Center
• Short Term Acute Care facility
• Team nursing model with higher ratios supported by multiple clinical staff

Compliance & Documentation Requirements

• Two forms of government-issued ID required (e.g., driver’s license, social security card, passport)
• Background check within 60 days of start including statewide, county, federal searches, and education verification
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and annually thereafter
• Separate sex offender search required
• Tennessee Adult Abuse Registry search required
• Physical exam within 1 year attesting ability to meet job demands
• Negative 10-panel drug test within 30 days of start (no pending compliance allowed)
• Fit test for 3M N-95 masks within 1 year (must pass without limitations; facial hair must be shaved if it causes failure)
• Immunizations required with proof:

  • Measles, Mumps, Rubella (2 vaccines or positive titer with boosters if low/equivocal)
  • Varicella (2 vaccines or positive titer with boosters if low/equivocal)
  • Hepatitis B (3-shot series, positive titer, or signed declination)
  • Seasonal Flu vaccine or approved medical/religious exemption (mask required if exempt)
• Negative TB test or chest X-ray within 1 year (2-step PPD, Q-Gold, or T-spot) with annual TB questionnaire if positive
• ACLS and BLS certifications required (AHA or ARC only)
• Photo for badge with strict guidelines (passport-style, no hats, no glasses glare, no filters, recent within 6 months)
• Two recent professional references required
• Skills checklist and internal evaluation required if employed with agency for 1+ year

Additional Notes

• Shift preferences (48 or 36 hours) should be indicated at submission
• No submission to multiple job IDs/facilities allowed; candidates must select one
• All Return to Office (RTO) restrictions must be included at submission; no changes after offer
• Dress code: Navy blue scrubs for RNs; no prints or embroidered scrubs allowed; business casual with lab coat for case managers
• Confidentiality acknowledgment and flu exemption forms must be signed prior to start
• EPIC questionnaire required for RNs only if applicable

This position is ideal for experienced LVN / LPN professionals with trauma and large hospital experience who have prior travel assignments and are ready for a night shift travel contract in a Level I Trauma Center environment.
