Case Manager (Registered Nurse) - Travel/Contract

Location: Sonora, California
Contract Length: 13 weeks
Shift: Day (5x8 hours), 7:00 AM – 3:00 PM
Hours per Week: 40


Job Summary

  • Plan, organize, and direct home care services to meet patient needs in their homes.
  • Manage a patient caseload efficiently and effectively.
  • Provide independent patient care and collaborate with other disciplines to meet care goals.
  • Serve as a resource to patients, family members, and care team members.

Required Qualifications

  • Valid Registered Nurse (RN) license in California.
  • Minimum of 1 year of case management experience, with case management experience within the last year.
  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
  • Neonatal Resuscitation Program (NRP) certification.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Experience in a Home Health setting.

Key Responsibilities

  • Complete initial and ongoing comprehensive nursing assessments of patients.
  • Implement and update individualized plans of care in collaboration with patients and care teams.
  • Provide direct and indirect skilled nursing care safely and effectively.
  • Document all patient care, status, and responses accurately and timely according to policies.
  • Educate and counsel patients and families on healthcare needs and preventive/rehabilitative procedures.
  • Comply with all legal, regulatory, and organizational standards including HIPAA and code of conduct.
  • May be assigned on-call duties based on agency needs.

Compliance & Documentation Requirements

Background Checks

  • Must have a background check within 30 days of start covering the last 7 years, including social security trace, county/state/federal court jurisdictions, OIG, OFAC, SAM, FACIS 3, National Sex Offender, and DMV/MVR (due to driving requirement).

Health & Immunization

  • Physical exam within 90 days of start (not annually required).
  • Drug testing within 30 days before start (specific panels required).
  • Immunizations or documented immunity/declinations required for:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Measles (Rubeola), Mumps, Rubella (positive titer, vaccines, or declination)
    • Hepatitis B (positive titer, series, or declination)
    • Varicella (positive titer, vaccines, or declination)
  • Negative TB test (QGold/TSpot or 2 PPDs) within 30 days of start, updated annually. Chest X-ray if history of positive TB.
  • Seasonal flu vaccine or facility-specific exemption/declination required during flu season (Oct 31 - Apr 30).

Competency Exams & Training

  • Medication Test/Pharmacology Competency Exam (Relias required).
  • Color Blind Testing (minimum 80% score).
  • Competency Exam (Relias required).
  • Dysrhythmia/EKG Competency Exam (annual, Relias required).
  • OSHA mandatory training (annual).
  • Additional certifications such as ACLS, PALS, NRP, TNCC, EFM, CPI, MAB, NIH Stroke Certification may be required depending on unit assignment (verify with recruiter).

Facility Specific Documents & Policies

  • Must review and sign facility-specific documents including confidentiality agreements, dress code, safety guides, tobacco policy, and others prior to start.
  • Passport-style color photo required for badge.
  • Must complete orientation and safety post-tests as required.

Additional Notes

  • Must comply with all vaccination requirements as a condition of employment; medical and religious exemptions may apply.
  • Reliable transportation and valid driver’s license are mandatory due to travel to patient homes.
  • On-call duties may be assigned based on operational needs.
