Job Opportunity: Home Health LVN / LPN

Location: Portage, Michigan
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Provide home health nursing care primarily for geriatric patients.
• Average caseload: 5-7 patients per day (to be confirmed during interview).
• Guaranteed 40 hours per week.
• Territory covers up to a 60-mile radius from the office hub.
• On-call required every 3rd to 4th weekend.
• Dress code: Scrubs of any color; jeans are not allowed.
• Documentation and communication must be timely and accurate.
• iPad provided for work; cell phone use is not allowed.
• Mileage reimbursement provided at IRS standard rate.
• Flexibility with travel within the territory is expected.
• First-time travelers are accepted; local candidates are also welcome at travel rates.

Required Qualifications

• Valid LPN / LVN license in Michigan.
• Minimum 1 year of recent Home Health experience within the last year.
• BLS certification issued by American Heart Association or American Red Cross.
• Experience in a Home Health setting.

Background and Compliance Requirements

• Comprehensive background checks required within 90 days of start, including:

  • SSN trace, county searches for last 7 years, nationwide sex offender, motor vehicle records, education and work history verification.
  • State-specific background checks as applicable.
• Fingerprinting required and must be current per state regulations.
• Education verification of highest degree related to profession.
• NURSYS license verification within 90 days of start.
• Drug screening (9-panel) within 12 months of start.
• Physical exam within last 12 months.
• Negative TB test or chest X-ray per facility requirements.
• Annual TB questionnaire required.
• Required immunizations or documented immunity for:

  • Measles (Rubeola), Mumps, Rubella
  • Hepatitis B
  • Varicella
  • Seasonal flu vaccination or facility-specific declination form during flu season.
  • COVID vaccination or facility-specific declination.
• Signed acknowledgment of Orientation Safety Manual and Acceptable Use Policy prior to start.
• Proof of current auto insurance required due to driving responsibilities.

Additional Requirements

• Must provide 2 professional references within 2 years of start date.
• Complete employment application with full 7 years of work history, including locations and no gaps.
• Annual skills checklist and competency exams as applicable.

• Candidates must be flexible with travel within the assigned territory.
• On-call weekends are mandatory as scheduled.
