Job Opportunity: Case Manager (Registered Nurse)
Location: San Leandro, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Must have 2 years minimum recent acute hospital experience in Clinical Case Management (CM) and discharge planning, specifically in an acute inpatient hospital setting.
- Experience limited to SNF, Rehab, or Hospice only is not acceptable.
- Must be willing to work weekend rotations.
Required Qualifications
- Minimum 2 years recent experience as a Clinical Case Manager with a heavy focus on discharge planning in an acute care hospital.
- Acute inpatient case management experience required.
- Current RN license for California.
- BLS certification (AHA only).
- Experience with InterQual, EPIC, and Allscripts/Careport systems.
Preferred Qualifications
- 4 or more years of experience preferred.
- Utilization Review experience is a plus.
- ACM or CCM Certification is a plus.
Prior Travel Experience
- No explicit requirement for prior travel experience mentioned.
Required Certifications & Documents
- BLS (Basic Life Support)
- ACLS (Advanced Cardiac Life Support) - AHA only (preferred)
- Other certifications such as PALS, NRP, TNCC, CHEMO, EFM, MAB, NIH Stroke, CPI may be relevant depending on unit but not explicitly required for this role.
- Current, unencumbered California RN license verification (NURSYS verification within 15 days prior to start).
- Background checks including criminal, EPLS/GSA/SAM, HHS/OIG within 90 days prior to start.
- Education verification of highest level completed.
- Physical exam within 90 days of start, signed by MD, DO, PA, NP or APRN.
- Fit mask test within 90 days of start (specific mask types required).
- Immunizations and health documents:
- Tetanus/Diphtheria/Tdap (within 10 years or signed declination)
- Negative TB test or chest X-ray updated annually
- MMR (Measles, Mumps, Rubella) proof of vaccination or positive titer
- Hepatitis B positive titer or declination
- Varicella proof of vaccination or positive titer
- Drug testing: Oral saliva drug screen within 30 days of start (rapid tests not accepted).
- Photo ID (Driver’s License or Passport) with clear, color photo.
- Employment history verification for past 3 years and current resume including last 7 years of employment.
- References per agency requirements.
- Completion of facility-specific competencies and orientation modules on first day of work.
- Seasonal flu vaccination or signed declination required by November 6; masking required during flu season regardless of vaccination status.
- Color blind exam within 90 days of start.
Additional Notes
- Must be willing to work weekends.
- Acute inpatient case management experience is critical; candidates with only SNF, Rehab, or Hospice experience will not be considered.
- Utilization Review and case management certifications are advantageous but not mandatory.
