Registered Nurse - Home Health (Travel/Contract)

Location: Chesapeake, Virginia
Contract Length: 13 weeks


Job Description

  • Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with periodic office meetings.
  • Average patient load: 5-8 patients per day.
  • Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from home clinic; average weekly travel 100-150 miles.
  • Schedule: Monday to Friday, 8 AM to 5 PM, with weekend rotations and on-call requirements varying by location.
  • Charting: Proficiency required in HomeCare HomeBase (HCHB) within the last 6 months; RNs must also be proficient in OASIS.
  • Dress code: Scrubs.

Requirements

  • Specialty: Registered Nurse - Home Health
  • Experience: Minimum 1 full year of Adult Home Health experience OR 1 full year Hospice experience (whichever is higher).
  • Prior Travel Experience: Not required.
  • First-timers: Not accepted; must have recent specialty experience.
  • Certifications: Current CPR and BLS required.
  • Licensure: Must have license in hand; pending licenses not accepted.
  • Transportation: Reliable transportation and current driver's license required. Agency-required liability insurance also required.

Productivity & Expectations

  • Weekly productivity expectation: 30 points (points assigned based on visit type and activities).
  • Patient assignment: 5-10 patients per day, equating to roughly 6-8 points daily.
  • No guaranteed hours; if patient census is low, contractor may be floated to nearby clinics to pick up hours.

Additional Information

  • Local candidates accepted at the same rate.
  • Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8, Monday-Friday.
  • Must be proficient with electronic medical records (HCHB and OASIS).
  • Background checks, drug screening, and various state-specific compliance documents required prior to start.
  • Candidates must complete multiple compliance and policy reviews, including HIPAA, confidentiality, and COVID-19 policies.
  • Physical exam and immunization documentation (including TB, MMR, Hepatitis B, Varicella, Flu, COVID) required within 12 months of start.
  • Two professional references and work history verification required.
