Job Opportunity: Physical Therapist - Home Health

Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide skilled physical therapy services in a home health setting.
  • Work 8-hour day shifts (no 4x10 or 3x12 shifts available).
  • Patient ratio is 1:1; may visit up to 6 patients per day.
  • Productivity standard: 30 points per day.
  • Must have at least 1 year of skilled home health experience.
  • First-time travelers are not accepted.
  • No weekend shifts required.
  • No on-call duties.
  • No floating between units.
  • Locals within a 50-75 mile radius are accepted.
  • Pending license holders are accepted.
  • Own transportation is required.
  • Occasional travel radius may exceed 50 miles.
  • COVID-19 primary series vaccination plus one booster required.
  • Experience with OASIS documentation is required.
  • Charting system: EPIC/OASIS.
  • Scrub color: Any.
  • Free parking available.
  • Mileage reimbursed at IRS standard rates.
  • Facility may call off up to one shift every two weeks.

Required Qualifications

  • Valid Physical Therapist license in Oregon (pending license accepted).
  • Minimum 1 year of Physical Therapist experience within the last year.
  • Experience in a Home Health setting.
  • Experience with OASIS documentation.
  • Basic Life Support (BLS) certification.

Additional Requirements

  • Must have own reliable transportation.
  • Must comply with facility’s Return to Office (RTO) restrictions (details to be confirmed).
  • Background checks and verifications as per state and facility requirements.
  • Physical exam within 1 year of start date.
  • Immunizations and health screenings required, including but not limited to:
    • TB test or chest X-ray
    • Tetanus/Diphtheria/Tdap
    • MMR (Measles, Mumps, Rubella)
    • Varicella
    • Hepatitis B or declination
    • Flu vaccine or declination (seasonal)
    • COVID vaccine or declination
  • Drug screening (10-panel) within 90 days of start.
  • Respirator fit testing required and must be updated annually.
  • Completion of required regulatory and specialty-specific training modules prior to start.
  • References: Minimum of 2 supervisor references covering 1 year of relevant employment within the last 3 years.

Notes

  • This position does not accept first-time travel therapists.
  • Weekend work is not required.
  • The facility does not accommodate block schedules or floating.
  • Locals within a 50-75 mile radius are considered and accepted at the same rate.
  • Facility requires adherence to specific background checks and state-specific searches.
  • Annual completion of required WorkRamp training modules is mandatory.
  • Physical exam and immunizations must be current and compliant with facility policies.
