Job Opportunity: Step Down Nurse (LVN / LPN)

Job Description

• Assignment in a Progressive Care Unit (Step Down)
• Schedule: 3 shifts per week, 12 hours each
• Floating required: You may be assigned to various units including High Risk Respiratory Unit/COVID and other specialty clusters as needed
• Must be available to work every other weekend and holidays
• Floating locations may include multiple campuses within a 15-mile radius
• Employer reserves the right to cancel shifts or the contract with notice due to census or other operational needs

Required Qualifications

• Valid Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in Ohio
• Minimum of 1 year of Long Term Care experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association
• National Tech Certification Verification
• Orientation Safety Manual Acknowledgement
• NIH Stroke Scale (NIHSS) competency
• Experience with floating to different units as assigned

Certifications & Competencies

• Basic Life Support (BLS) - AHA certification required
• Dysrhythmia/EKG Competency Exam: Must be passed within 1 year prior to start or completed shortly after contract signing (3 attempts allowed)
• Medication Test/Pharmacology Competency Exam within 2 years of start
• Other certifications may be required depending on unit assignment (e.g., ACLS, PALS, NRP, TNCC, EFM) but not mandatory for this role unless specified by unit

Background & Health Requirements

• 7-year criminal background check including SSN trace, national and county checks, sex offender registry
• Drug screening including marijuana panel within 12 months prior to start
• Physical exam within 2 years indicating fitness for duty and free of communicable diseases
• Immunizations or proof of immunity for:

  • TB (negative test or chest x-ray and questionnaire if previously positive)
  • Measles, Mumps, Rubella (titer or vaccination)
  • Varicella (titer or vaccination)
  • Hepatitis B vaccination series, immune titer, or declination
  • Seasonal flu vaccination or approved medical/religious exemption (declinations not accepted)
• Respiratory safety clearance within 1 year of start, renewed annually

Documentation & Compliance

• Government-issued photo ID required
• I-9 and E-Verify completed prior to start
• Education verification (Primary source) required for all non-RN candidates
• Skills checklist updated within 1 year prior to documentation due date
• Employment application or current resume required
• Two professional references within 2 years (one must be a manager/supervisor/charge nurse) or employment verification if references unavailable
