Job Opportunity: Physical Therapist - Home Health
Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Seeking a Physical Therapist for home health services in Medford, OR and surrounding areas. The position involves visiting up to 6 patients per day with a 1:1 patient ratio and standard productivity expectations.
Work Schedule
- Shift: 8-hour day shifts
- No weekend requirement
- No on-call duties
- No floating between units
- No block scheduling available
- Locals within 50-75 mile radius considered local
Experience & Requirements
- Minimum 1 year of skilled home health experience required
- First-time travelers not accepted
- Basic Life Support (BCLS) certification required
- Pending license accepted
- Traveler must have own transportation
- Facility requires primary series COVID vaccination plus one booster
- Experience with OASIS documentation required (please note if no OASIS experience)
Facility & Work Environment
- Type of Facility: Home Health
- Attire: Any color scrubs allowed
- Charting system: EPIC/OASIS
- Parking: Free
- Mileage reimbursement: IRS standard
- Facility may call off up to one shift per pay period (every two weeks)
Required Qualifications
- Valid Physical Therapist license for Oregon (pending license accepted)
- BCLS certification
- Skilled home health experience (minimum 1 year)
- COVID vaccination (primary series + booster)
- OASIS documentation experience
Additional Compliance & Documentation Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Drug screening: 10-panel within 90 days of start (no instant tests)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health documents required:
- Tetanus/Diphtheria/Tdap (within 10 years or declination)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination during flu season
- COVID vaccine or declination
- Respirator fit testing required annually (travel only)
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application covering at least last 5 years
Training & Competency
- Completion of required WorkRamp modules (regulatory, specialty, and regional) prior to start and annually as required
- Competency exams and medication/pharmacology tests as applicable
- EPIC training modules completion required within a week of orientation
Notes
- Prior travel experience is required (first-time travelers not accepted)
- Traveler should be prepared for occasional travel radius up to 50+ miles for patient visits
- Facility may have restrictions on guaranteed hours (may call off one shift per pay period)
