Job Description

Required Qualifications

• Valid Registered Nurse (RN) license in New York (must be verified within 30 days of start)
• Minimum 1 year of Operating Room (OR) experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association
• Trauma Nursing Core Course (TNCC) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• State License 1 Verification
• NURSYS verification within 30 days of start

Background and Compliance Requirements

• 7-year background check completed within 30 days of start, including:

  • Social Security Trace
  • County search
  • National Sex Offender Registry
  • National Criminal Background
  • FACIS Level 3
  • OFAC
  • NY Medicaid Exclusion List (OMIG)
  • OIG and SAM checks
• EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start
• State-specific Medicaid Exclusion List updated annually

Health and Safety Requirements

• Facility-specific physical exam completed by a physician within 30 days prior to start (must use facility form)
• Fit test for N95 mask (Halyard Fluidshield or 3M 1860) within 12 months of start
• Negative TB test or chest x-ray within 1 year of start
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (antibody and antigen results required; proof of vaccine series alone is insufficient)
• Seasonal flu vaccination required October 1 – April 30 (declinations allowed for religious or medical reasons)
• COVID vaccine series preferred
• Standard 9-panel drug screen completed within 30 days prior to start (no rapid tests)

Competency Requirements

• Medication/Pharmacology competency exam with minimum score of 80%
• Additional competency exams may be required depending on unit (e.g., Dysrhythmia/EKG)

Documentation and History

• 2 clinical references required (supervisor level or above; charge RNs acceptable)
• Employment history covering 7 years with explanations for any gaps
• Current skills checklist applicable to the position, updated within the last year
