Job Description

Weekend Requirements: Minimum of 4 weekend shifts in a 6-week period.

Certifications Required

ACLS: Advanced Cardiac Life Support
BLS: Basic Life Support
PALS: Pediatric Advanced Life Support
NRP: Neonatal Resuscitation Procedure
CHEMO: Chemotherapy
EFM: Electronic Fetal Monitoring
NURSYS Verification: Required for all RNs
IV Certification: Required for LVN/LPN/Other
Management of Assaultive Behavior (MAB): Required
NIH Stroke Certification: Required
CPI: Crisis Prevention Institute
Medication Test/Pharmacology Competency Exam: Required as applicable by profession

Additional Requirements

Background Check: Required within 45 days of start, covering all locations lived/worked within the past 7 years.
Physical Exam: Must be signed by a licensed healthcare provider, indicating the candidate is free of infectious disease.
Drug Testing: 10 Panel Drug Screen within 90 days of start.
Vaccinations:
- Tdap Vaccine
- Flu Vaccine (during flu season)
- COVID-19 Vaccination or Declination
References: Minimum of 2 references covering 1 year of employment total within the last three years.

Skills and Competencies

Competency Exam: Required for applicable unit and must be updated annually.
Skills Checklist: Required for applicable unit and must be updated annually.

Oregon Neglect Abuse Consent Form: Required for candidates going to specific facilities.
