Minimum Requirements
<ul><li>Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.</li><li>Active Connecticut Physical Therapy license required to start the assignment.</li><li>BLS certification from AHA or ARC may be required.</li><li>Experience in home health delivery preferred. Candidates must be able to manage caseloads in patients' homes and document visits electronically.</li><li>Valid driver's license and reliable transportation required for travel between patient homes. Mileage between visits will be reimbursed.</li><li>Minimum education requirement: Bachelor's Degree. Degree in Physical Therapy required.</li><li>Ability to work independently and coordinate care across interdisciplinary teams. Previous experience with home health documentation and care planning preferred.</li></ul>
