Licensed Practical Nurse (LPN) Position - New Hampshire

Job Details

• Location: Berlin, New Hampshire
• Contract Length: 13 weeks
• Shift: Day (7 AM - 3 PM) and Evening (3 PM - 11 PM)
• Hours per Week: 48
• Facility Type: Long-Term Care (LTC) and Skilled Nursing Facility (SNF) with 100 beds (96 semi-private, 4 private) across 2 floors
• Patient Population: LTC/SNF patients with diagnoses including diabetes, COPD, post-stroke, heart disease, fractures, dementia, Alzheimer’s, rehab needs, chronic conditions, and multiple comorbidities
• Staff Ratios: Approximately 1 LPN to 17-20 patients
• Scheduling: Schedule made 6 weeks in advance; flexibility required to cover staffing gaps
• Weekend/Holiday: Every other weekend required; may be asked to work holidays
• Floating: Ability to float between floors required
• Dress Code: Professional appearance required; any color scrubs, no large jewelry, no fake nails, hair must be up, no face tattoos

Job Responsibilities

• Medication administration and passes
• Vital signs monitoring
• Blood sugar checks and management
• Wound care, ostomy, and Foley catheter management
• Tube feeding administration
• Assisting other team members as needed
• Occasional management of IV antibiotics and PICC lines
• Respiratory treatments and equipment management (CPAP, nasal cannula, high flow oxygen); no tracheostomy or ventilator care
• Participation in daily team huddles

Required Qualifications

• Valid LPN/LVN license in New Hampshire
• Minimum 1 year of Long Term Care experience within the last year
• Experience in a Skilled Nursing Facility setting
• Current BLS certification

Additional Information

• Electronic Medical Records (EMR) system used alongside paper charting
• Interdisciplinary team includes PT, OT, SLP, case managers, social workers, and dieticians
• Physician coverage includes nurse practitioner rounds M-F mornings and MD availability evenings and bi-monthly visits; after-hours coverage via hospital on-call provider
• Pharmacy uses bubble packs and maintains house stock of generic medications
• Orientation includes 3 shifts with a preceptor
• No call or standby requirements; covered by permanent staff

This position offers a structured LTC/SNF environment with a diverse patient population and requires a flexible, experienced LPN comfortable with a variety of nursing duties and occasional IV management.
