Location: Rancho Cordova, California
Contract Length: 13 weeks

Job Description

Seeking an experienced Home Health Physical Therapist with at least 2 years of experience. The role involves providing physical therapy treatments in home environments, assisted livings, senior apartments, and board and care homes. Patient caseload typically includes 3 to 5 visits per day, including 2 Start of Care visits daily. Diagnoses treated include orthopedic, cardiovascular, pulmonary, oncology, neurological, geriatric, and amputations. No pediatrics or hospice care.

Schedule & Work Environment

• Shift: 8:30 AM to 5:00 PM
• One weekend per month requested but not required
• Holiday work required, with some flexibility for time off requests
• Work environments may be cramped, hot/cold, or cluttered
• Dress code: Business professional; scrubs recommended but not provided; closed-toe shoes required; no jeans, tank tops, or shirts with logos
• Supervision of up to 2 PTAs or COTAs
• Robust 3-week orientation including online learning and field precepting; solo work expected by end of week 2 or 3

Required Qualifications

• Minimum 2 years of experience in healthcare; home health experience including OASIS preferred
• Active California Physical Therapist license
• Current BLS certification (AHA)
• Active NPI number
• Valid driver’s license and proof of automobile insurance
• Good oral and written communication skills; strong organizational skills
• EMR HomeCare Home Base (HCHB) experience preferred

Additional Information

• Candidates must not have been employed full-time, part-time, or PRN by certain affiliated health systems within the past year; PRN employment through outside firms is acceptable
• Mileage reimbursement available following specific guidelines (mileage begins after first patient visit)
• Patient territories are grouped to reduce excessive mileage and improve efficiency; flexibility to cover different teams as needed
• Managed by an RN Manager with support from Regional Rehab Leader/Educator

Notes

• No other certifications required beyond BLS and valid PT license
• No prior travel experience explicitly required
• Caseload weighted by visit type to account for travel, documentation, and case management time
