RN Case Management / Discharge Planning

Location: Jackson, Mississippi
Contract Length: 13 weeks

Job Summary

Seeking an experienced RN Case Manager specializing in discharge planning for inpatient units at a Level 1 or large academic medical center hospital. The role involves managing medical discharges with a caseload of 15-20 patients per day, working Monday through Friday, 8 AM to 4:30 PM, with no weekend requirements and holiday rotations as needed.

Requirements

• Specialty: RN Case Management / Discharge Planning
• Years of Experience: Minimum 2 years of acute care management experience in discharge planning
• Additional Experience: At least 6 months experience at a Level 1 or large academic medical center hospital working with various inpatient units
• Certifications:

  • BLS required
  • CCM preferred but not required
• Education: Prefer RN with BSN; ADN considered
• EMR Experience: EPIC charting experience is required
• Shift: 5 days per week, 8-hour shifts (Monday-Friday)
• Caseload: 15-20 patients per day
• Prior Travel Experience: Not specified as required

Job Details

• Department: General Case Management / Discharge Planning
• Patient Population: Medical discharges primarily, some social discharges
• Coverage: All inpatient units throughout the hospital
• Team:

  • 15 current case managers, goal to expand to 21
  • Works closely with a social work team (15 full-time, 17 part-time)
• Supervision: Reports to Social Work Manager; oversight by Chief of Care Coordination
• Orientation: 2 weeks including general facility orientation; will work alongside CM team (no solo work)
• Collaboration: Consults with Respiratory Therapy and Pharmacy as needed for discharge planning
• Dress Code: Any color scrubs or business professional attire

Additional Information

• Must comply with a 50-mile radius rule for travelers
• Flu vaccine required (medical exemptions allowed with documentation)
• Fingerprint background check required (remote or in-person options)
• No weekend shifts; holiday rotations required
• Understanding of discharge readiness criteria needed (no specific InterQual or Milliman experience required)
• Facility is the only Level 1 trauma center in the state, handling complex patients and those with limited hospital options
