RN Case Manager - Home Health

Location: Lebanon, New Hampshire (with service areas including Bradford, VT; Randolph, VT; Springfield, VT; White River Junction, VT; Lebanon, NH)
Employment Type: Contract
Contract Length: 13 weeks
Shift: Days
Hours per Week: 36

Job Description

Provide comprehensive case management for home health patients, including screening, evaluation, re-evaluation, and treatment.
Develop and periodically review patient care plans in collaboration with physicians, patients, and caregivers.
Coordinate care through interdisciplinary communication and case conferences.
Monitor and assess patients’ clinical, functional, and emotional status, communicating significant changes to physicians and team members.
Document evaluations, treatment goals, and objectives accurately.
Educate patients and caregivers on self-care techniques, medication, diet, and provide health counseling as appropriate.

Required Qualifications

Valid Registered Nurse (RN) license in New Hampshire.
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Minimum of 2 years of experience in Home Health nursing, including experience within the last year.
Experience working in a Home Health setting.
