Job Opportunity: PT Home Health (Therapy/Rehabilitation)
Location: San Jose, California
Employment Type: Travel/Contract
Position Details
- Specialty: Physical Therapist (PT) and Speech-Language Pathologist (SLP) in various counties including Sonoma, Marin, Contra Costa, Alameda, Santa Clara, San Joaquin/Stanilaus, and Sacramento.
- Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
- Visits: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
- Work Schedule: No weekends required; no mandatory on-call duties (on-call is discretionary)
- Locals Accepted: Yes, within a 50-mile radius
- Dress Code: Any color scrubs, closed-toe shoes
- Charting: Must be completed onsite using Kinnser/Wellsky; no charting from home
- Parking: Free residential parking available
Experience & Requirements
- Years of Experience:
- Minimum 1 year in Home Health
- Minimum 2 years overall as a clinician (higher value applies)
- First-Time Travelers: Accepted
- Licensing: Must have valid state license and BLS certification at time of submission (no pending licenses accepted)
- Certifications Required:
- BLS (Basic Life Support)
- State License
- ACLS and PALS preferred (American Heart Association or American Red Cross)
- Contract Length: 4-6 months (less experienced clinicians must commit to 6 months, non-negotiable)
Compliance & Documentation Requirements

- Government-issued photo ID (Driver’s License/State ID)
- Emergency Contact Form
- Background Checks (within 1 year, including SS Trace, County searches, Nationwide Sex Offender, EPLS/GSA/SAM, HHS/OIG)
- Certifications (BLS, ACLS, PALS as applicable)
- State License Verification (primary source online verification)
- Personal or Auto Insurance
- Physical Exam (within 6 months, signed by MD/DO, free of active communicable diseases)
- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (within 10 years or exemption)
- Negative TB test or Chest X-Ray (within 1 year)
- Measles, Mumps, Rubella (proof of immunity or vaccination)
- Hepatitis B (titer, declination, or vaccine series)
- Varicella (titer, vaccination, or history)
- Drug Testing: 9 Panel Drug Screen within 1 year
- Flu Vaccination or Declination (required during flu season, 10/1 - 3/31)
- COVID Vaccination or Medical/Religious Exemption
- References:
- 2 professional references for travel, allied, or per diem acute care
- 1 professional reference for per diem non-acute care (not required for non-clinical roles)
- Skills Checklist: Required annually
Additional Notes
- Mileage reimbursed based on IRS standard from first patient to last patient (not to/from home)
- Territory/County coverage will be discussed during interview; may include surrounding counties
- All required documents must be submitted before starting patient care; compliance processing may take 2-3 weeks
- Return to Office (RTO) requests must be submitted at time of application; no additional RTO accepted after offer
- Desired clinician qualities: patient care focused, punctual, self-starter, diligent with charting (tablet provided)
