Required Certifications

• Negative 10-panel drug screen within 30 days prior to start
• Respiratory Safety Clearance or Respirator Questionnaire as applicable
• References: 2 supervisory references or performance evaluations within 1 year of start
• Pediatric skills checklist as applicable

• Flu vaccination is mandatory for clearance; exemptions require agency-specific forms
• Medication calculation/dosage test during orientation requires 100% pass rate with up to three attempts
